What you need to know

Life happens, and when it does, your benefits change with you. Make sure to contact the Fund Office when any of the following life events occur, so you may enroll new dependents or make changes to your benefits elections as needed.

Marriage/Divorce

Your spouse is automatically eligible for benefits on the date of your marriage. To enroll your spouse, contact the Fund Office within 30 days after the marriage for an enrollment form to be effective on the date of your marriage.

If you and your spouse get legally separated or divorced, your spouse will no longer be eligible for coverage. COBRA Continuation Coverage is available for your spouse for up to 36 months if your spouse wishes to continue coverage under the Plan. You must notify the Fund Office within 60 days of your legal separation or divorce date in order for your spouse to enroll in COBRA.

You may also want to review or update your beneficiary information for your Life and AD&D benefits if you get married or divorced.

Adding a Child

Your natural child will be eligible for coverage on the date of their birth. Coverage for adopted children, children in placement for adoption, or foster children in placement with you will begin on the date of their placement. You must be responsible for their health care coverage, and the child must meet eligible dependent requirements to be eligible for coverage. Stepchildren that live with you will become eligible for coverage on the date of your marriage.

Contact the Fund Office within 30 days of the event to request a form for their enrollment in coverage.

Child Loses Eligibility

Your child is no longer eligible for benefits once they reach the age of 26. At that time, you must contact the Fund Office to notify the Fund of this event. Coverage continuation under COBRA is available for up to 36 months as long as you or your child notify the Fund Office within 60 days of when the child turns 26 to enroll.

If your child turns 26 but is disabled due to physical handicap or mental disability, and cannot engage in daily living activities or paid employment/work, coverage may continue under the Plan as long as your coverage continues and they continue to depend on you for care and support. Your child must be disabled before they turn 26 to qualify. COBRA Continuation Coverage is also offered for disabled dependents.

Death

In the event of your death, your designated beneficiary will receive a Life Insurance benefit (and an AD&D Insurance benefit if your death was accidental).

Your spouse and eligible dependents will remain eligible for coverage for a period of three consecutive months after your death. COBRA Continuation Coverage is available for your spouse and eligible dependents for up to 36 months as long as they continue to make the necessary self-payments.

Continue Retiree Coverage

Your surviving spouse and dependents can continue coverage under retiree coverage if you meet the following requirements:

  • At least 55 and an eligible active participant in the Fund at the time of your death
  • Earned at least five Vesting Credits with the Milwaukee Drivers Pension Trust Fund

As long as the above criteria is met and premium payments are made, your surviving spouse can continue retiree coverage for life. Eligible dependents can continue their coverage as long as they remain eligible under the plan and premiums are paid for them.

If the above criteria is not met or your spouse waives or opts out of this coverage, the surviving spouse and eligible dependents will be allowed a three-month extension of benefits after your death. Once the three-month extension is up, your surviving spouse and eligible dependents can elect and pay for COBRA Continuation Coverage for up to 36 months.

Other life events include end of work, out of work due to disability, FMLA, military duty, retirement, and the death of a dependent. Refer to your Summary Plan Description for more information.
