What you need to know
Before enrolling in health and welfare benefits for you and your dependents, you must first meet the eligibility requirements set by the Milwaukee Drivers Health and Welfare Trust Fund. After meeting initial eligibility for benefits, you’ll continue to be eligible under the Plan as long as contributions are made on your behalf.
Initial Eligibility
Milwaukee Drivers Health and Welfare Trust Fund provides health care and welfare coverage for eligible employees and their dependents. You become eligible for benefits on the first day of the calendar month following:
- Five consecutive weeks in which your employer has contributed to the Fund on your behalf or
- The month in which your employer has contributed to the Fund for each week during that month
Once you’re officially an active participant in the Milwaukee Drivers Health and Welfare Plan, you’ll need to enroll to gain access to our comprehensive benefits package, which includes:
Retirees

Dependents
Your dependents may become eligible for benefits on the day you become eligible or on the date you add a dependent. You must notify the Fund Office to add a dependent for benefits coverage. If you do not notify the Fund in a timely manner, eligibility for your dependent will begin on the first day of the month following the date you notified the Fund.
Eligible dependents include:
- Your legal spouse (coverage starts date of marriage)
- Your child(ren), from the day of their birth or placement with you until the last day of the month in which they turn 26. This includes your natural children, children who are adopted or placed with you for adoption, stepchildren, and foster children.
Note: Coverage for disabled children aged 26 and older may be available if the disability occurred before the dependent turned 26. You may also continue coverage through COBRA. Refer to your Plan Documents for further details.
Continued Eligibility
Once you meet the eligibility requirements, you will continue to be eligible for benefits under the Plan as long as your employer continues to make contributions to the Fund on your behalf. Contributions must be made each week of the month, two months before the month you’re eligible to continue receiving benefits coverage under Plan. Refer to the chart below for details.
Note: If your employer is late in making contributions to the Fund, the Trustees may, at their discretion, continue your eligibility in the Plan for up to three months. You may also continue your eligibility by making self-payment contributions to the Fund if your employer cannot. Refer to your Plan Documents for more information.
